Student Accommodations
Accommodation Process
Students are invited to voluntarily identify themselves as having a disability to initiate the accommodation process.
- Fill out the Student Disclosure of Disability form (.pdf). Students may identify themselves as having a disability at any time.
- Upon receipt of the form, Equal Opportunity Services will contact the student to secure the appropriate current documentation (within the last five years) and/or testing results needed to verify the disability. (See Documentation Guidelines)
- EOS will work with the student and members of the campus community to arrange reasonable accommodations.
- EOS will also prepare an accommodation memorandum for distribution to the faculty. The Instructor Accommodation Memorandum documents the disability and describes the accommodations to be made. They are valid for one semester.
- All information will be kept confidential and EOS will contact campus community members only on a need-to-know basis.
