Faculty and Staff Accommodations


In compliance with government regulations, Carnegie Mellon makes reasonable accommodations to faculty, staff and students of the university with disabilities. If you believe you are covered by the American with Disabilities Act of 1990, the Rehabilitation Act of 1973 or the Vietnam Era Veterans Readjustment Act of 1974, you are invited to voluntarily identify themselves as having a disability to initiate the accommodation process.

  1. Fill out the Employee Voluntary Self-Identification form (.pdf). Employees may complete this form at any time.
  2. Upon receipt of the form, Equal Opportunity Services will contact the employee to secure the appropriate current documentation (within the last five years) and/or testing results needed to verify the disability. (See Documentation Guidelines)
  3. EOS will work with the employee and members of the campus community to arrange reasonable accommodations.
  4. All information will be kept confidential and EOS will contact campus community members only on a need-to-know basis.

Refusal to provide information will not subject an applicant or employee to any adverse treatment. The provision of this information will provide documentation of a disability and may qualify individuals for certain accommodations in their physical work environment in accordance with applicable laws and regulations.
