Vision Coverage Options
Davis Vision and Vision Benefits of America (VBA) are the administrators of our vision programs. Your vision selection is normally a two-year election. The election you make during the 2008 Open Enrollment will be in effect through December 31, 2009, unless you experience a life/family status change. Full-time, benefits-eligible faculty and staff may participate in the vision options, which are designed to give you flexibility in choosing your coverage.

Vision Benefits Information
Davis Vision Plan Information - Search for a provider, review benefits, view frame collection, learn about laser vision correction surgery or the Lens 1-2-3 program, or take a customer satisfaction survey. (See also the Davis Vision Schedule of Benefits (.pdf))
Obtaining Vision Care: During an appointment with a participating provider, simply tell the provider that you are a Davis Vision member, and the provider will submit the claim to Davis Vision. When using a non-participating provider, you should pay for the service in full, and then submit a claim for reimbursement at the out-of-network level.

Obtaining Vision Care:
Many participating vision care providers can submit electronic claims, so that participants do not need to request a benefit form in advance. The “Search Providers” tool of the VBA website provides information about providers equipped for electronic claims.When using providers without the ability to file electronic claims, VBA participants must request a benefit form before seeking services from a participating provider for the visit to be considered in-network. If you do not get a benefit form prior to your appointment, the visit will be considered out-of-network. Request a benefit form from the VBA website or by calling VBA’s Customer Service at 1-800-432-4966 before making an eye appointment. A personalized benefit form will be mailed within a few days. You may also pick up a benefit form and VBA Participating Provider list in person at the South Hills VBA office by calling and requesting it in advance.
When seeking out-of-network services, pay for the service in full at the appointment. Then, request and submit a benefit form and itemized receipts to VBA for out-of-network reimbursement.
Additional Vision Services: HMO Benefits and Eyetique Discount
The HMO programs include limited vision benefits. HealthAmerica HMO utilizes the EyeMed Vision Care network (866-723-0514 or http://www.eyemedvisioncare.com. Check your plan documents or contact the carrier for details about the vision benefits included in the HMO plans.
Staff Council, through its participation in the Pittsburgh Employees Activities Association, has made a discount program available through Eyetique. Employees, their dependents and immediate family can receive up to a 20% discount on exams and glasses/contacts. Discounts also coordinate with your vision insurance benefits, and a 10% benefit for audiology service is available through Eartique. See the Staff Council PEAA for more information.
