Filing Claims with EBDS
Starting in 2008, Employee Benefit Data Services (EBDS) administers the HCFSA accounts for all those who participate. (In 2007, HealthAmerica participants used CBSA. CBSA claims submitted after December 21, 2007 should be submitted to EBDS for processing due to the end of our contract with CBSA.)
Caremark (formerly PharmaCare) prescription expenses will be automatically submitted to EBDS for you. Medical expenses for Highmark participants will also be automatically submitted to EBDS. If you would prefer not to use auto-submission and to file all claims manually, use the EBDS website (http://wwww.ebdsbenefits.com/eosa) to turn off this feature.
To submit claims to EBDS:
- Save the Explanation of Benefits form that you receive from your insurance carrier or the receipt from the provider/store if this expense was not processed through an insurance plan. (See the Summary of Acceptable Documentation.)
- Complete sections 1, 2 and 4 of the first page of the EBDS FSA Claim Form.
- Mail the materials to: Account Service Center, P.O. Box 22128, Pittsburgh, PA 15222.
Or FAX them to: 412-394-9669 - EBDS will process the claim and mail you a check within about two weeks (minimum check value is $25). You can also establish Direct Deposit or review your claims history at the EBDS website.
- Access your account at the EBDS website at http://www.ebdsbenefits.com/eosa. Your Social Security Number will be your Customer ID. Your PIN number – which you can change after you initially log in – was sent to you in a letter from EBDS. You can contact the EBDS customer service department at 1-800-524-5503 if you cannot locate your PIN.

2007 HealthAmerica Health Plan Participants: CBSA
The Flexible Spending Account (FSA) carrier for HealthAmerica participants in 2007 was Corporate Benefit Services of America (CBSA). HealthAmerica medical expenses were submitted to CBSA automatically. As of December 21, 2007, EBDS will begin processing the 2007 claims of CBSA participants. If you have money remaining in your 2007 FSA account, you have until June 30, 2008 to file claims with EBDS for those funds.
If you submit your claim to CBSA, it will be forwarded to EBDS for processing, which will result in a delay in payment. For the quickest service, please submit claims to EBDS as of December 21, 2007. Please see the section above for information about submitting claims to EBDS.
