Statement of Health
High levels of life insurance require you to demonstrate your good health by providing medical evidence of your good health, through a MetLife Statement of Health (SOH) Form (.pdf).

In most cases, the SOH requirement can be satisfied by completing a medical questionnaire. If medical problems are indicated, the insurance company may require you to complete a longer questionnaire or have an examination by a physician. During Open Enrollment, you will be directed to the EOI medical questionnaire if it is required - you must complete and submit it promptly. The carrier will contact you if you need to answer additional questions or obtain a medical exam to satisfy medical EOI.
Acceptance of your statement of health will be determined by the carrier in accordance with its guidelines governing medical underwriting. The carrier determines the effective date of coverage once the medical review has been conducted. Approval may take up to several months. During this period, a new employee or dependent will be insured for the Guaranteed Issue amount. For existing employees, coverage will stay at your prior level of coverage until an underwriting decision is made. You will only be charged the monthly rate for the life insurance coverage you are currently receiving.
* Note: changes in spousal coverage due to a change in salary will not require a SOH. Changes in spousal coverage due to electing additional levels of spousal coverage will require a SOH.
